Please submit this form to advise us that you intend on applying for either the APP or CPP initial or renewal certification. Once your information is received, a personal file folder will be created for you on our secure PSCBW Google Drive. An access link to your folder will be emailed to you. You will only have access to that one folder. Your main folder will contain a separate folder for each individual document required for your application. You may begin uploading your documents as you receive them.

Once your folder contains everything required for your application, use the email below to notify us that your application is completed and ready for our review process.
